Registering as a new patient
We welcome requests for registration from patients living in or moving to the Practice area, which serves Swanage and the surrounding area. Please note the Practice catchment area on the map below and enter your postcode to check whether your address falls within the area we serve. Registration in person
You can download a copy of our Patient Registration Form (DOCX, 595KB) to print out and complete. Alternatively, collect a paper copy from reception.
Please return the completed form to the Practice, where you will be asked for identifying documentation.
Identifying documentation (ID)
You do not need proof of ID to register with a GP, but it would help if you have one or more of the following:
- Passport
- Birth Certificate
- HC2 Certificate
- Rough sleepers identity badge
- Hostel or Accommodation registration or mail forwarding letter
- If you are homeless, you can give a temporary address, such as a friend's address, a day centre or the GP surgery address.
After you are registered with us
To maintain an even distribution of patients among the GPs on registration, you will be allocated a specific GP who will have overall responsibility for the care and support our surgery provides you.
If you do not return your registration form in person, you will be allocated a GP and registered at our Practice within 24 hours.

Registering as a Temporary Patient
You can register as a temporary resident with the Practice for up to 3months. This can be helpful if you're living away from home but do not want to change your home GP surgery.
To register you'll need to fill out a temporary services form (GMS3), available from the surgery.
After 3 months, you'll have to re apply to register as a temproary resident or become a permanent pateint.
