Swanage Medical Practice are working with Dorset County Hospital to ensure that patients with Type 1 diabetes are receiving appropriate offers of education and support to manage their condition.     

We will therefore be sharing information with them about our Type 1 Diabetes patients who have been referred for education sessions so they can consider offering additional help and support to those individuals.
